HomePatient access

Your next step, made simpler.

Book a visit, access your portal, or find the paperwork and billing guidance you need. You do not need to do everything at once.

Start where you are

One place. Three clear choices.

Scheduling and portal registration are separate steps. If you have already booked, keep that appointment and use the portal for the next steps our practice assigns.

Find a visit

Book an appointment

Use Zocdoc as our main online booking route. Not sure about your coverage or the right visit? Ask us before booking.

New to the portal

Request portal access

Request access to our PracticeQ patient portal. Our practice reviews registration requests before approving access.

Request portal access
Already registered

Sign in to PracticeQ

Use your existing portal account and the secure instructions from our practice. Please do not register a second account.

Patient portal sign in

Portal access is not an appointment or acceptance for treatment. Please keep the optional registration note administrative; save medical history for your assigned secure forms. For help, call or text 508-978-6455.

Prepare for care

The right paperwork, not every form.

Our practice will tell you which intake and consent documents to complete. Please do not fill out both intake versions unless we ask you to.

Assigned by our practice

Full intake

A more detailed history for an evaluation, when requested. Use the secure form invitation sent to you; this page does not start an unassigned intake.

Use only when requested

Brief intake

A shorter intake option when our practice tells you it is appropriate. It is not an automatic replacement for the full intake or a follow-up visit.

About consent: You can request portal access or ask about an appointment before completing a treatment packet. Before care begins, we will guide you through the applicable treatment and telehealth consents, privacy notice, and financial policies. A portal account or submitted history does not replace informed consent. For anyone under 18, contact our practice first so we can arrange the appropriate consent and guardian-access process.

Understand the cost

Out-of-network care, step by step.

If we confirm that the out-of-network pathway is appropriate, you pay the agreed fee and may ask your plan to reimburse you. Coverage and reimbursement are not guaranteed.

  1. Confirm your payment pathway

Ask us whether your specific visit will use accepted insurance, out-of-network benefits, or self-pay. If your visit is billed through Headway, follow its billing instructions; do not pay a second full-visit invoice or submit a duplicate claim.

  2. Check benefits before you commit

Ask your insurer about out-of-network outpatient mental health coverage, telehealth coverage, your remaining deductible, allowed amount, reimbursement percentage, any prior authorization, and the claim deadline. Save the reference number from your call.

  3. Review fees and complete assigned paperwork

We will confirm the fee and applicable financial documents before care. If you are uninsured or not using insurance, ask about your written Good Faith Estimate. A posted fee list is not a patient-specific estimate.

  4. Attend your visit and pay through the confirmed route

Follow the payment instructions given by our practice. Registering for the portal does not authorize a charge or establish insurance coverage.

  5. Request your superbill securely

After the visit and payment, request your itemized superbill through the secure portal or Spruce. Our practice will review the service and diagnosis codes, provider details, and payment record before releasing it. If you cannot find it, contact us rather than submitting an incomplete document.

  6. Submit to your insurer and track its response

Follow your plan's member-claim instructions and include any additional claim form it requires. Keep the submission confirmation and Explanation of Benefits. Contact your insurer about coverage decisions; contact us securely if a document needs correction. We do not currently submit out-of-network claims for you.

Protect your privacy: A superbill contains diagnosis and service information. Submitting it shares that information with your insurer. Do not send superbills, insurance cards, or clinical records through ordinary email or the public website contact form. If you have Medicare or MassHealth, contact us before booking or choosing self-pay; this general out-of-network pathway is not clearance to bypass program rules.

A little clarity

Before you click.

I already booked on Zocdoc. Do I need to book again?

No. Keep your existing appointment. Portal registration and intake paperwork are separate from scheduling. If anything looks inconsistent, contact our practice before making another booking.

Does completing an intake mean I have an appointment?

No. An intake supplies information for review. It does not reserve a visit, guarantee acceptance for care, establish coverage, or guarantee a prescription.

Do I need to complete both the full and brief intake?

No. Complete only the version our practice assigns. If you have already submitted information, tell us so we can check it before asking you to repeat it.

Can I download a superbill immediately?

Please request it securely after your visit and payment. Availability depends on the billing pathway and completion of our review. We will tell you how to access the released document; submitting it does not guarantee reimbursement.

What if I cannot access the portal?

Use the password-recovery option on the portal sign-in page, or call or text 508-978-6455 for administrative help. Do not send a password, medical history, or insurance documents by ordinary text.
