Read the website termsRequest portal access or use the secure invitation from our practice. Not every person receives every form. The forms assigned depend on age, service, payment arrangement, consent needs, and the information required for the evaluation.
Use the PracticeQ portal or the secure form invitation provided by our practice. We will identify whether the full or brief intake is appropriate and provide the applicable consent documents. Please do not complete both versions unless requested.
Reviewing public policies is not the same as signing an assigned consent. Portal registration and completed forms do not schedule an appointment or guarantee acceptance for care. Contact us first about paperwork for anyone under 18.
Use the secure scheduling route to review current availability. For self-pay, language-access, form-access, or other nonclinical administrative questions, call or text 508-978-6455. Do not include medical information in an ordinary text or email.
A requested appointment change is not final until a new date and time are confirmed. Scheduled in-person and telehealth availability is confirmed directly with the practice.
